How to Authorize an Agent or Property Manager

Use this form to authorize either specific people or corporations, such as a property management company, to file and complete Rent Control documents on behalf of a property owner. The owner must sign this form.

The following steps will help you authorize an agent for your property

  1. Step 1: Get the Application

    You may download an application here. You can fill the type directly on the form, but you will need to print it when you have finished. If you would rather get a paper application from us, please email us at rentcontrol@santamonica.gov or call (310) 458-8751 during business hours to request that an application be mailed to you.

  2. Step 2: Identify the property for which you would like to authorize an agent.

    Enter the site address of the property on Line 1, including the street number and street name. If the unit is a condominium, please be sure to include the unit ID.

  3. Identify yourself as the owner of this property

    Because you are authorizing someone to complete registration-related documents on your behalf, it is important you enter your name on Line 2 and agree to be bound by each document filed by the agent you are authorizing to the same extent as if you had filed the document and executed it under penalty of perjury yourself. 

  4. Identify the individual and/or property management company you wish to authorize.

    Enter the full name of the person you wish to make your authorized agent on Line 3 of the form. If you would like to authorize a property management company and each of it's employees, then you may enter the name of the company on Line 4 and leave Line 3 blank. 

    Enter contact information for the authorized agent as requested on Lines 5, 6, 7 and 8.

  5. Check the box to opt-in to Rent Control news and announcements

    Just like property owners, it is important for authorized agents to understand the rent control law, opting in to occasional news and announcements from us will help them stay up-to-date.

  6. Select one recipient for all official Rent Control mail and bills.

    Currently, all formal notifications, mail, and bills are sent by mail, and owners must identify where that mail should be sent. Only one official mailing address may be chosen. If you would like to have all official mail sent to the agent you are authorizing with this form, please check the first box in Section 9 - "Agent/Property Manager Address Above." If you would like us to continue to send mail only to you or another owner on record, please check the second box in Section 9 - "Owner on Record." In this case, we will not change the mailing address. If you have another person or property management company already registered and you want all official mail to continue to be sent to them, then check the third box in Section 9 - "Other Authorized Agent/Manager on Record"

    If you need to change your mailing address or that of another owner of the property, please file a Change of Mailing Address form. 

  7. Print the form and complete the declaration

    Print the form and sign the declaration affirming that all information stated on this form is true and correct with a pen. 

    Please enter your own name, even if title to the property may be held by an LLC, corporation, or trust. Then, sign and date the form. If the property has more than one owner, only one owner needs to sign the form. 

  8. Mail the form to us

    Since we need your original signature on the document, please mail it to us at the address below when it is complete.
