How to Obtain a Copy of a Paramedic Report
For patients treated and or transported by the Santa Monica Fire Department, medical records may be requested by the patient or his/her authorized representatives. Medical records are confidential and protected by Federal and State law which require that your medical records and protected health information (PHI) remain confidential. Therefore, the following guidelines must be strictly followed to allow release of medical records.

Request By Patient or Authorized Representative of Patient
+ -Complete the Authorization for Release of Protected Health Information Form by clicking here.

Drop off the Authorization for Release of Protected Health Information request form.
+ -Bring the Authorization for Release of Protected Health Information request form and a photo ID to the Santa Monica Fire Department Administration Offices at 333 Olympic Drive, Second Floor, Santa Monica, CA. 90401, Monday through Thursday between 8:00 a.m. and 4:00 p.m.

Subpoenas or Law Office Requests
+ -Subpoenas can be served in person at the above address Monday through Thursday between 8:00 a.m. and 4:00 p.m., by U.S. mail, or email. All subpoenas must include the $15.00 subpoena fee. Records will not be released until the $15.00 fee payment is received in full. All subpoenas will be reviewed by the City Attorney prior to release of records.
